Job Summary
Monitor daily biometric readings from connected devices, review alerts for abnormal trends, and contact patients to address concerns. Provide education on device usage and medication adherence while coaching patients on healthy lifestyle habits. Coordinate device delivery, setup, and troubleshooting, then escalate clinical findings to providers per established protocols. Assist with patient enrollment, track compliance, and collaborate with care teams to ensure continuity. Participate in quality improvement initiatives to increase engagement and reduce hospitalizations. Stay current on CMS guidelines and organizational workflows within the eClinicalWorks EHR environment.
Required Qualifications
- active South Carolina RN, LPN, or CNA license
- strong customer service and patient engagement skills
- strong technical abilities
- understanding of RPM practices
Desired Qualifications
- previous RPM experience preferred
- experience with chronic care management, primary care, cardiology, etc. preferred
