Job Summary
Remote Patient Monitoring - LVN/LPN supports patients remotely by handling RPM alerts, conducting outbound and inbound calls, and coordinating care with providers. Responsibilities include monitoring RPM data, addressing health concerns, routing non-clinical inquiries, documenting interactions in the clinical platform, completing monthly wellness assessments, and maintaining compliance with policies. Must maintain an active LVN/LPN license, be proficient with EMRs/telehealth tools, and meet the 4-day, 10-hour shift schedule with required weekend coverage. Training lasts five weeks (Mon-Fri, 9:00 AM–6:00 PM ET). Compensation includes base pay of $24-$26/hour plus differentials and bonuses, with full benefits and a 1,500 sign-on bonus after 120 days. AI fluency and use of AI tools are non-negotiable expectations for daily productivity and clinical documentation support.
