Job Summary
Remote Nurse fluent in English, Mandarin and/or Cantonese to support Home Health patients and caregivers. Responsibilities include contacting caregivers to confirm patient visits, ensuring the Plan of Care is current, transmitting care plans and related documentation, and accurately documenting interactions in the EMR. Maintain HIPAA compliance, coordinate between field staff and office personnel, and assist with basic care coordination and administrative nursing tasks from a remote, home-based setting. Requires NY RN license and multilingual proficiency (English, Cantonese and/or Mandarin). Hours aligned to US EST; remote work from home with tools and reliable connectivity required.
Required Qualifications
- Registered Nurse with New York License - USRN - NY
- Fluency in Cantonese and/or Mandarin and English required
- Previous experience in Home Health, Care Coordination, Case Management, or Remote Nursing preferred
- Strong verbal and written communication skills
- Experience using EMR/EHR systems
- Excellent organizational and documentation skills
- Ability to work independently in a remote environment
- Proficient with email, phone communication, and basic computer systems
