Job Summary
Provide remote patient support via phone, video, and secure messaging; conduct initial health assessments and follow-up check-ins; educate patients on treatment plans, medications, and preventive care; document patient interactions accurately in electronic health records; collaborate with physicians and other healthcare providers to ensure seamless care; monitor patient progress and escalate urgent cases as necessary; maintain confidentiality and comply with HIPAA and other healthcare regulations.
Required Qualifications
- Licensed Registered Nurse (RN) or equivalent certification
- Strong communication skills and ability to build trust remotely
- Experience with telehealth platforms and electronic health records preferred
- Reliable internet connection and quiet workspace
- Ability to work flexible hours (part-time with potential to transition to full-time)
