Job Summary
Remote Healthguide role supporting patients and their primary care teams from a fully remote setting, with 10-15% compensated travel to clinical sites within the Columbus, GA area. Responsibilities include coordinating with medical practice staff and Guidehealth’s Case Management team to improve patient health outcomes, documenting patient interactions in the EHR, navigating referrals and prior authorizations, implementing high-impact care plans, closing care gaps, escalating issues to nurses or physicians, and maintaining strong patient-provider relationships. Requires Certified/Registered Medical Assistant or Certified EMT with 2+ years of experience, proficiency with multiple EHR systems (eClinicalWorks, EPIC, Athena, Cerner), excellent written/verbal communication, and the ability to work independently in a remote environment while meeting KPIs.
Required Qualifications
- Certified or Registered Medical Assistant OR Certified EMT with 2+ years of experience
- Willingness to become proficient in motivational interviewing and behavioral change management
- Experience with documenting in EHRs (eClinicalWorks, EPIC, Athena, Cerner) and to balance priorities
- Strong communication, empathy, and professional demeanor
- Ability to work independently in a remote environment and to travel 10-15% within Columbus, GA area
- Reliable transportation and valid driver's license for occasional site visits
- Adherence to patient confidentiality and PHI/PII handling policies
