Job Summary
Build trusting relationships with patients, families, and providers while engaging high-risk populations through calls, texts, video visits, and AI agents. Assess medical, behavioral, and social needs to support whole-person care, educate patients on care plans, and navigate medical and social systems. Conduct nonclinical assessments, identify trends, and escalate clinical concerns using structured workflows. Manage referrals, coordinate follow-ups, and assist with appointments and prior authorizations to reduce avoidable hospitalizations. Document all interactions in Epic, Athena, or Cerner systems and support risk adjustment and quality measures like eCQM and HEDIS.
Required Qualifications
- Must be fully bilingual in Arabic and English, including full medical terminology
- Must hold a valid Medical Assistant Certification
- Equivalent experience includes working in similar roles for several years without formal certification
- Demonstrated experience with EHR documentation and remote-work effectiveness
- Must be able to lift 50 lbs
Desired Qualifications
- At least 2–3 years of related experience in patient navigation, medical assistance, health promotion, care coordination, or similar roles
- Experience working directly with patients in clinical or non‐clinical settings, preferably with complex medical/social needs
