Job Summary
Coordinate MDS scheduling and care plan development with interdisciplinary teams to ensure compliance with RAI guidelines and federal regulations. Provide Medicare, Medicaid, and Managed Care oversight to verify clinical services and secure appropriate reimbursement. Develop individualized resident care plans, review quarterly updates, and verify documentation to support clinical standards. Ensure timely submission of Minimum Data Sets to state databases while promoting quality improvement through data analysis and trend identification.
Required Qualifications
- Nursing Experience in MDS Assessment: 3+ year
- RN required
- Thorough understanding of PDPM requirements
- Able to negotiate through EMR and possess strong computer skills
Desired Qualifications
- RAC-CT preferred
- Promotes and demonstrates excellence in customer service
