Job Summary
Remote, full-time LPN/LVN to manage patient census, identify and coordinate community resources, provide patient education on chronic-condition management, perform medication management and reconciliation, ensure timely delivery of services (home health, DME, home infusion), and assist with referrals and scheduling. Requires an active multi-state/compact LPN license, 3+ years of patient-facing experience, strong communication skills, and comfort working remotely with HIPAA-compliant workspace; Epic experience and bilingual abilities are a plus. Opportunities for production-based advancement and scheduling are noted, with a Monday–Friday daytime schedule and occasional on-call.
Required Qualifications
- Active Multi-State/Compact LPN license (NLC)
- LPN/ LVN licensure in good standing
- 3+ years direct patient-facing experience
- Ability to work fully remote with home office setup
- Proficiency with Microsoft Office Suite
- Strong written and verbal communication skills
