Job Summary
Remote Intake Coordinator role for an LPN (NLC/LVN) delivering value-based care management. Responsibilities include accepting transfers from the Patient Enrollment team to conduct preliminary health assessments, resolving patient questions, identifying and coordinating community resources for chronic care management, providing patient education and health literacy on chronic-condition management, assisting with medication management and coordinating refills, and ensuring timely delivery of services (Home Health, DME, Home Infusion, etc.). Requires fast-paced adaptability, most of the shift on the phone, and an active LPN license with multi-state/compact credentials; remote-work setup with HIPAA-compliant home office and high-speed Wi-Fi.
Required Qualifications
- Active Compact/Multi-State license (LPN)
- Technical aptitude – Microsoft Office Suite
- Remote work readiness with HIPAA-compliant workspace
- Strong written and verbal communication skills
- Epic Experience (preferred)
- Bilingual (preferred)
- Additional Single State licensures (preferred)
