Remote Intake Coordinator - Licensed Practical Nurse (LPN) - $21/hr - Day Shift

Remote Intake Coordinator role for an LPN with 3+ years of direct patient-facing experience. You will accept transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients, resolve questions, and create dialogue to understand needs. Identify and coordinate community resources to benefit patient care, provide patient education on chronic-condition management, and assist with medication management including coordinating refills. The role supports home health, DME, home infusion, and other critical needs, with most of the shift spent on the phone. Requires an active Compact/Multi-State LPN license, Microsoft Office proficiency, and a HIPAA-compliant home workspace. Remote position, Full-Time, Monday–Friday with no weekends and a once-a-year on-call rotation. Benefits include health insurance, 401k with company match, paid holidays, PTO, and company laptop.
