Job Summary
Nurse Practitioner to join Galileo's Population Health team providing telephonic and video-based follow-up care for patients recently discharged from hospital, with a focus on medication reconciliation, symptom screening, and coordinating follow-up labs, medications, and services. responsibilities include building trust with patients, explaining Galileo’s care model, and linking patients to needed services; coordinate care across hospital/ED transitions, pharmacies, and community providers; collaborate with RN and administrative colleagues; maintain accountability to individual and team performance targets; part of a 24/7 digital-first primary care model with 32-36 hours/week; weekend shifts required on a monthly basis; compensation at $78/hour (regular) and $90/hour (premium); active SC NP licensure required; optional additional state licenses; 5+ years in Primary Care or Geriatrics; bilingual English/Spanish bonus.
Required Qualifications
- Certified Nurse Practitioner
- Active state licensure in SC
- ability to obtain additional state licensures
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- 2 of those years may be substituted by time spent working at a Skilled-Nursing facility or Long Term Care facility
- Direct experience engaging Medicare and Medicaid populations
- Motivational interviewing skills
- Comfort in a digital-first environment
- Solid tech proficiency; familiarity with Google Suite and Slack is a plus
- Excellent written and verbal communication skills
- Bilingual in English and Spanish is a strong bonus
