Remote Case Manager - Integrated Care

Coordinate care for members as part of an interdisciplinary team; perform screening/assessment, develop and monitor Person-Centered Service Plans, manage risk stratification and transitions of care, and serve as primary point of member contact. Ensure timely documentation and data reporting, apply care guidelines and Medicaid knowledge, educate members, and collaborate with PCPs, caregivers, providers, and community supports. Occasional field visits (1–2/week) in the Fort Lauderdale area; requires computer literacy and ability to navigate multiple systems. Temp-to-perm, in-person role with standard 8-5 hours.
