Job Summary
Remote Care Coordinator role for a Licensed Practical Nurse (LPN/LVN) focused on value-based and chronic care management. Responsibilities include managing patient census, identifying and coordinating community resources, providing patient education on chronic condition management, medication management (reconciliation, adherence, and coordinating refills), ensuring timely delivery of services (Home Health, DME, Home Infusion), resolving patient questions, and managing referrals and appointment scheduling. Must hold an active multi-state/compact LPN license, have strong communication skills, and be proficient with Microsoft Office. Remote, full-time, Monday–Friday with no weekends and rare on-call. Epic experience and bilingual ability are pluses. Salary starts at $21/hour and can go up to $28/hour based on production; benefits and 401k available.
Required Qualifications
- Active Compact/Multi-State LPN/LVN license
- Proficiency with Microsoft Office Suite
- Excellent written and verbal communication skills
- Minimum 3 years direct patient-facing experience
