Remote Call Center Nurse - Evening or Night Shift

Job Summary
Answer incoming calls and place outbound calls to patients, facilities, pharmacies, and hospitals while verifying identity per HIPAA guidelines. Document all telephone encounters accurately in the electronic medical record, route calls based on urgency, and escalate urgent clinical concerns immediately. Coordinate same-day provider visits, schedule follow-up appointments, and assist with medication refill requests, laboratory results, and imaging reports. Serve as a liaison between providers and skilled nursing, assisted living, and long-term care facilities to ensure timely resolution of patient care needs. Manage voicemail queues, track open telephone encounters, and complete administrative duties as assigned.
Required Qualifications
- Associate degree or higher
- Current and active Registered Nurse (RN)
- Current and active multi-state license as Licensed Practical Nurse (LPN)
- Knowledge of medical terminology
- Proficiency with data entry and electronic health record
- Excellent verbal and written communication skills
- Ability to prioritize
- Excellent customer service
- 1-3 weekends per month 3pm to 11pm or 11pm to 7am
- At times, travel is required to companywide meetings
Desired Qualifications
- Current and active multi-state license as Registered Nurse (RN)
- Experience in skilled nursing, post-acute care, care coordination, or case management
- Familiarity with data entry, medication reconciliation, discharge planning and community resources
- Familiarity with quality measures and tracking
- Strong interpersonal, communication, and patient advocacy skills
- Ability to manage multiple priorities in a fast-paced environment
- Knowledge of post-acute workflows and care transitions
- Strong analytical, organizational, and problem-solving skills
