Job Summary
Monitor individuals for health and safety while safeguarding their dignity and rights. Safely evacuate residents during fires or disasters and physically escort or restrain them during emergencies. Assist individuals with bathing, transfers, and personal hygiene tasks, including lifting those weighing 150–250 pounds. Educate residents on goals specified in their ISP and behavior plans, assist with meal preparation, and accompany them to places of worship. Maintain daily records, document progress toward DHSP objectives, and complete incident reports. Perform housekeeping duties such as laundry, dishes, and snow shoveling. Work a flexible schedule including weekends and holidays, with a minimum of three shifts per month.
Required Qualifications
- High School Degree or G.E.D.
- Must be 21+ years old
- Valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- First Aid, CPR & Safety Care certifications
- Certification in Applied Non-Violence
- Certification in Safety Care
- Certification in PBS
- Ability to work up to twelve hours at a time
- Ability to lift and/or transfer/move individuals
- Ability to physically assist individuals in bathing and personal hygiene tasks
- Ability to physically assist individuals in transfer and carries
- Ability to safely evacuate individuals in the event of fire or disaster
- Ability to remain on duty after scheduled hours if not relieved
- Ability to work a minimum of 3 shifts every month
- Ability to work a flexible schedule including weekends and holidays
- Ability to be reassigned from one program to another without prior notice
- Ability to communicate with the Residential Logistics Director at least once every two weeks
- Ability to maintain confidentiality
- Ability to be an active listener
- Ability to utilize clear and direct communication
- Ability to work effectively and cooperatively with individuals, staff, families, community members and others
- Ability to consistently incorporate individual's therapeutic goals into daily activities
- Ability to document each person's progress towards DHSP objectives
- Ability to complete all necessary data sheets for therapists
- Ability to complete incident reports as necessary
- Ability to implement Agency policies and procedures
- Ability to maintain confidentiality and positive demeanor while juggling multiple priorities
- Ability to display professional attitude and behavior
- Ability to demonstrate a high regard for each person and sensitivity for cultural values
- Ability to focus on providing consumer satisfaction as primary goal
- Ability to assist individuals to identify, develop and participate in meaningful activities
- Ability to support individuals in making and expressing choices
- Ability to identify and provide support needed by individuals to become part of the community
- Ability to follow the learning style and implementation strategy as outlined in each person's DHSP
- Ability to educate and train individuals in goals and objectives specified in ISP and behavior plan
- Ability to educate and assist individuals with meal preparation
- Ability to educate, transport, accompany and provide supervision to individuals in/to their place of worship
- Ability to physically escort/restrain individuals during emergency situations or behavioral episodes
- Ability to ensure and safeguard individuals' human dignity, rights, and liberties
- Ability to monitor individuals for health and safety
- Ability to maintain records and document daily data
- Ability to handle related job duties in compliance with the Agency Policy and Procedure Manual
- Ability to provide emergency coverage to include overnights and direct care coverage
- Ability to coordinate and assist with planning of leisure time
- Ability to perform daily housekeeping and cleaning of residence
- Ability to use the restroom
- Ability to regularly lift and/or transfer/move individuals in and out of wheelchairs, vehicles, up from and down to a mat
- Ability to regularly lift and/or transfer/move individuals weighing 150 - 250 pounds
