Job Summary
Rehabilitation Specialist for Transitional Age Youth (TAY) in a full-time dayshift role located in Frederick, MD. Responsibilities include engaging young adults through relationship development and person-centered planning, tailoring supports to promote independent living and successful transition to adulthood, coordinating services across the agency and community, participating in team meetings and case conferences, coordinating assessments and program planning, and maintaining required documentation according to program standards. The role requires strong collaboration with families, school personnel, and referral sources; use of EHR and Microsoft Office; and a willingness to obtain additional certifications. The program emphasizes safety-net supports, outcome-focused care, and engagement in meaningful activities such as school, volunteer work, or paid employment. The pay rate is $19.31 hourly with benefits and tuition reimbursement opportunities.
Required Qualifications
- High school diploma or GED
- Two years positively referenced relevant work experience with Transitional Age Youth (TAY) in an educational or social service setting and/or general vocational knowledge
- Bachelor’s degree in a related field preferred
- Driver’s license with 3-points or less and access to an insured vehicle
- Willingness to obtain additional certifications and trainings as required
- Availability to work days, nights, weekends, and holidays per program needs
- Proficiency with Microsoft Office and electronic health record systems
