Job Summary
Rehabilitation Specialist at Sheppard Pratt, a direct-care role within the Segue Residential Transition Program, providing support for mental health treatment and helping clients transition out of state psychiatric hospitals. The role involves participating in initial assessment, intervention, and re-evaluation; working one-on-one to develop independent living and socialization skills; coordinating comprehensive care including monitoring medication; providing transportation to medical appointments and advocating for follow-up instructions; and contributing to a supportive, mentorship-rich team environment. Awake overnight shift is Thursday–Saturday from 10pm to 8am, with responsibilities across daily living activities, treatment planning, and client advocacy.
Required Qualifications
- A high school diploma or GED
- A driver’s license with 3-points or less and access to an insured vehicle
- Must have availability to work days, nights, weekends, and holidays depending upon need of the program
- The willingness to obtain additional certifications and trainings as required
- Bachelor’s degree in a related field is preferred
