Job Summary
Conduct initial assessments, interventions, and re-evaluations of client needs while working one-on-one to help individuals learn or regain skills for independent living and socialization. Coordinate comprehensive care for assigned clients, including monitoring medication compliance, and provide transportation to medical appointments with full advocacy and documentation of follow-up instructions. Maintain a safe, therapeutic environment that encourages the overall independence and wellbeing of clients transitioning from state psychiatric hospitals. This direct care role operates Tuesday through Saturday from 2pm to 10pm under close supervision with supportive mentorship from senior staff.
Required Qualifications
- High school diploma or a GED
- A driver's license with 3-points or less and access to an insured vehicle
- Must have availability to workdays, nights, weekends, and holidays depending upon need of the program
