Job Summary
Educate team members on community activities and facilitate member connection to employment and rehabilitation services. Assess, coordinate, link, and monitor supportive, skills-based services in a community setting using the Vocational Activity Profile. Participate in developing individual service plans and initiate or coordinate services related to ISP goals. Complete referrals in a timely manner while becoming familiar with referral processes. This role requires a current AZ Driver's License, clean MVR, and BHT certification. We offer generous PTO, comprehensive medical benefits, and a 401K match.
Required Qualifications
- Highschool diploma or GED
- 1 year of experience working with individuals with serious mental illness (SMI)
- Current AZ Driver's License (valid and in good standing)
- Clean 39-month Motor Vehicle Record (MVR)
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
- Fingerprint Clearance Card
Desired Qualifications
- Associates degree in the field of social sciences
- Familiar and knowledgeable of available community resources
- Minimum of six months of recovery from substance use and/or mental health disorders
- Peer Certification (if applicable)
