Job Summary

Rehabilitation Specialist needed at 23rd Ave Health Center in Phoenix to assist clients with identifying treatment and recovery goals, implement services per Individual Service Plans, coordinate rehabilitation-related supports, document progress, and apply Motivational Interviewing and supportive listening within an interdisciplinary team to support recovery and employment goals.

Required Qualifications

  • An associate’s degree in behavioral health, or high school diploma or GED with a minimum of 3 years of behavioral health experience, or
  • A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or
  • A master’s degree or higher in the field of behavioral health
  • 1 year of supervised experience as a Rehabilitation or Employment Specialist, or Vocational Rehabilitation Counselor preferred
  • Valid AZ Driver License, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
  • Must have a valid Arizona Level 1 Fingerprint Fingerprint Clearance card or be able to apply for one within 7 working days of starting employment
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Basic understanding of the DSM-V preferred
  • Must pass background check, TB test and other pre-employment screening
  • Minimum typing speed of 25 wpm
