Job Summary
Determine required rehabilitation assessments to clarify participant needs and coordinate services. Develop thorough rehabilitation plans in consultation with participants, families, and Veterans Affairs Canada case managers. Act as the service integrator by collaborating with medical, psycho-social, and vocational professionals to deliver safe, cost-effective, and high-quality care. Empower participants to problem-solve by exploring community-based interventions and resources to achieve desired outcomes. Identify barriers or risk factors contributing to suboptimal outcomes and pro-actively develop and implement strategies to reduce or eliminate them. This role supports veterans of the Canadian Armed Forces and their families in re-establishment across Western Canada.
Required Qualifications
- Bachelor's degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field
- A health professional designation
- Certified Vocational Rehabilitation Professional (CVRP or a CVRP-F)
- Minimum of (3) year experience in the direct provision of medical, psycho-social or vocational rehabilitation services
- Experience working with persons living with mental or physical health problems creating barriers in life
- Employment conditional on obtaining federal security clearance – Reliability Status
- Ability to travel across the region as required
Desired Qualifications
- Bilingual (French / English)
- Experience working with employers, community resources, and training institutions
- Current and former members of the Canadian Armed Forces (CAF)
