Job Summary
Determine required rehabilitation assessments to clarify participant needs and coordinate services aligned with program mandates for improving functional well-being. Develop thorough rehabilitation plans in consultation with participants, families, rehabilitation professionals, and case managers. Act as the service integrator by collaborating with medical, psycho-social, and vocational specialists to deliver comprehensive care. Empower participants to problem-solve by exploring community-based interventions and resources while identifying barriers to proactively implement strategies. This role supports veterans of the Canadian Armed Forces and their families from a work-from-home position in Québec with minimal travel.
Required Qualifications
- A bachelor's degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field
- A health professional designation
- Certified Vocational Rehabilitation Professional (CVRP or a CVRP-F)
- Career Counsellors registered with the Ordre des conseillers et conseillères d'orientation du Québec
- A minimum of (3) year experience in the direct provision of medical, psycho-social or vocational rehabilitation services
- Experience working with persons living with mental or physical health problems creating barriers in life
- Employment conditional on obtaining federal security clearance – Reliability Status
- Ability to travel across the region as required
Desired Qualifications
- Bilingual (French / English)
- Experience working with employers, community resources, and training institutions
- Current and former members of the Canadian Armed Forces (CAF)
