Job Summary
Conduct thorough assessments of patients' physical limitations to design personalized treatment plans aligned with recovery objectives. Implement therapeutic exercises targeting strength, balance, and coordination while supporting patients in relearning essential mobility skills like walking and transferring. Educate families on fall prevention strategies and administer pulmonary rehabilitation programs to improve lung function and exercise capacity. Collaborate actively with nursing and occupational therapy staff to ensure cohesive care, assisting patients in improving daily living activities and introducing cognitive rehabilitation techniques. This full-time role at Alveo Aged Care's onsite Rehabilitation Center in Merced focuses on helping elderly residents regain independence following surgeries, strokes, or chronic illnesses.
Required Qualifications
- A valid Physical Therapy license in the state of California
- A Bachelor's Degree in Physical Therapy or a Doctor of Physical Therapy (DPT) degree from an accredited institution
- Strong communication and interpersonal skills to facilitate effective interaction with patients and team members
- The ability to develop and execute patient-centered treatment plans tailored to individual needs
Desired Qualifications
- A minimum of 2 years of clinical experience in a skilled nursing facility
