Rehabilitation Counselor On-Call

Job Summary
Provide care and supervision to assigned clients to ensure a safe, healthy, secure, and pleasant environment that promotes growth and development. Facilitate client independence in self-care, daily living skills, socialization, and community living while participating in and modifying individual service plans. Track client progress, provide supplemental reports, and serve as an advocate encouraging a proactive role in the rehabilitative process. Offer guidance and support by teaching skill development in nutrition, basic self-care, and routine medical care. Maintain a hazard-free environment and possess the ability to perform physical tasks including lifting, kneeling, and reaching. This role supports the Behavioral Health Services Division at Horizon House, where candidates may be required to report during emergencies or inclement weather.
Required Qualifications
- High School Diploma
- Associate's Degree preferred in the social sciences or a health care-related field, and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required
- Good communication skills, both oral and written
- Ability to work independently and as part of a team
- Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents
- Possess manual dexterity and fine motor skills
- May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs)
- May require crisis intervention in emergencies
- May be required to report to work during emergencies, including inclement weather
Desired Qualifications
- Associate's Degree preferred in the social sciences or a health care-related field
