Job Summary
Provide care and supervision of assigned residents to ensure a safe, healthy, secure, and pleasant environment that promotes resident growth and development. Assist in designing and implementing Individual Program Plans while documenting client progress as they move through the service delivery system. Advocate resident growth toward independence in self-care, daily living skills, socialization, and community living. This role requires the ability to lift, carry, kneel, and reach, with potential direct intervention during behavioral incidents. Applicants must possess a degree in social sciences or health care with one year of relevant experience in mental health or mental retardation fields. On-call rotation may include reporting during emergencies or inclement weather. Horizon House, a pioneer in community-based rehabilitation, serves over 4,500 adults with psychiatric or developmental disabilities across Eastern Pennsylvania and Delaware.
Required Qualifications
- degree in the social sciences or health care related field
- one year of relevant experience in the MH/MR field
- prior experience with implementing behavior management plans and/or supports
- a combination of experience, education or training that would provide the level of knowledge, skill and ability required
- Very good communication skills, both oral & written
- Ability to work independently and as part of a team
- Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents
- Possess manual dexterity and fine motor skills
- valid drivers' license
- acceptable driving record
