Job Summary
Assess patients' need and support systems for efficient discharge planning, psychosocial, counseling, and social drivers of health support needs. Monitor and update interdisciplinary care plans, establishing plans for effective management of identified episodic and population-based needs. Perform assessment, planning, intervention, and reassessment in a collaborative environment with Medical Staff, Clinical Case Managers, Nursing, Allied Health disciplines, hospital business/financial services, and other stakeholders. Requires a master's degree in social work and minimum three years of acute care hospital experience. Background screening through The Florida Care Provider Background Screening Clearinghouse is mandatory.
Required Qualifications
- Master's degree in social work (MSW) from a Council of Social Work Education (CSWE) accredited university
- Minimum of three (3) years of acute care hospital or other related healthcare experience
- Minimum of two (2) years of acute care hospital case management experience
Desired Qualifications
- Active Case Management Certification (CCM, ACM-SW, A-SWCM) or within one (1) year of hire and membership in relevant CM organization
- Demonstrated ability to manage high risk situations, multiple tasks and adjust priorities according to patient needs and department/hospital needs
- Strong ability to communicate effectively both verbally and in written work, and when on-call to respond to issues, patient needs
- Strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare team, payers and external agencies
- Solid clinical social work assessment, critical thinking, problem-solving, interviewing, decision-making, negotiation, conflict management and organizational skills
