Job Summary
Assess residents through assessments, care plans, discharge planning, and grievance resolution while managing MDS documentation and volunteer coordination. Oversee monthly activity calendars and scheduled programming to ensure psychosocial support for long-term care residents. Resolve customer service issues and maintain effective communication with families and staff. Requires experience in long-term care, knowledge of the MDS/RAI process, and proficiency with Point Click Care EHR. Full-time role with competitive compensation and comprehensive benefits.
