Job Summary
Provide evidence-based rehabilitative nursing care to adult patients in a hospital setting. Assess patients, develop and manage individualized care plans, and coordinate with an interdisciplinary rehab team. Support mobility and activities of daily living, assist with safe discharges, document care in the EHR, educate patients and families, and champion quality and safety in a supportive, teamwork-driven culture.
Required Qualifications
