Registry/PRN - Care Manager - Social Worker/ LSW/ LCSW/ Care - IP Care Management

Care Manager for IP Care Coordination at Shirley Ryan AbilityLab; communicates with the clinical team and third-party payors to assess, develop, coordinate, advocate, and monitor a care plan that utilizes the patient's resources in the most appropriate and cost effective manner. Evaluates assessment information, assigns patients to the appropriate clinical path, and monitors with the physician and clinical team. Coordinates patient care by facilitating clinical team conferences, overseeing the ordering of durable medical equipment (DME) and supplies, and ensuring completion of the treatment plan. Initiates and maintains communication with external case managers and primary care physicians to obtain pre-authorization for ongoing rehabilitation needs and advocate for the patient to receive appropriate rehabilitation services. Negotiates conflicts with third-party payor representatives on behalf of the patient and Institute; participates in planning and execution of Shirley Ryan AbilityLab sponsored educational programs. Requires nursing or social work education with relevant licensure/certification and strong communication/advocacy skills; CRRN strongly preferred for nursing track and MSW for social work track; Equal Employment Opportunity Employer.
