Job Summary
Interdisciplinary team-based, full-time permanent role in the TRANSITIONS program (Hamilton, ON) providing on-site and in-home short-term case management and referrals to mental health, addictions, physical health, and related services. Responsibilities include assessments, care planning, coordinating care, documenting, and facilitating referrals to a range of community and social services; health promotion, advocacy, information sessions, and resource development to support tenants in navigating health and social systems. Requirements include a degree in Social Work, a Diploma in Nursing (RPN), or Recreation Therapy (or related discipline) with registration to a regulated college, experience in case management, trauma-informed and culturally safe practice, knowledge of local services, collaboration with an interdisciplinary team, and a valid G-class driver’s license with own vehicle. Hours: 40 per week; on-call one week every five weeks; location includes on-site and in-home work within the Greater Hamilton Area. Required Qualifications
- Degree in Social Work, Diploma in Nursing (RPN), Recreation Therapy or a related discipline
- Registered with a regulated college of professionals or willing to get registered
- Experience with case management for individuals with serious mental illness, addictions, or other needs
- Person-centered & trauma-informed approach
- Knowledge of community and social services in the Hamilton area
- G class driver’s license with own vehicle
