Registered Social Worker / Dementia Care Clinician

Job Summary

Provide comprehensive psychosocial assessments, supportive counselling, and therapeutic interventions for individuals living with dementia and their care partners. Develop individualized care plans and manage case navigation for families experiencing complex needs. Participate as a member of interdisciplinary Primary Care Memory Clinic teams to support early diagnosis and coordinated care. Facilitate psychoeducational support groups and advocate for clients by connecting them with community resources. Maintain clinical documentation and contribute to continuous quality improvement while traveling throughout the four-county region. Requires a valid Ontario Class G driver's licence, ability to work in office, clinic, community, and home settings, and flexibility for occasional evening or weekend events.

Required Qualifications

  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) from an accredited university
  • Current registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) as a Registered Social Worker (RSW)
  • Minimum three years of experience providing clinical social work services
  • Experience conducting psychosocial assessments, supportive counselling, care planning, and case management
  • Experience working with older adults, individuals living with cognitive impairment and their families
  • Excellent communication, relationship-building, and organizational skills
  • Valid Ontario Class G driver's licence
  • Access to a reliable, insured vehicle
  • Vulnerable Sector Police Check (within the last 2 months)
  • Ability to work in a combination of work in office, clinic, community, and home settings
  • Flexibility in scheduling, including the ability to begin work earlier or end later than regular office hours (8:30 a.m. – 4:30 p.m.) to support travel time to clinics/programs a few times each month
  • Ability to attend occasional evening or weekend events

Desired Qualifications

  • An MSW and experience in dementia care, geriatrics, primary care, or community-based healthcare
