Job Summary
Provide social work services to individuals, couples, families, and organizations in a community setting to enhance social functioning and offer counseling, therapy, and referrals. Establish, implement, evaluate, and adjust goal-oriented service plans in collaboration with clients, families, caregivers, and care coordinators. Promote client safety by identifying risks and responding appropriately, while maintaining confidentiality of personal health information per provincial regulations and college standards. Work within a multi-disciplinary team of healthcare professionals. This casual, unionized role is based in St. Thomas and requires a Master's or Bachelor's degree in Social Work with current active registration.
Required Qualifications
- Registered Social Worker
- Master's degree or bachelor's degree in Social Work from an accredited university
- Current active registration and in good standing with Ontario College of Social Workers
- Knowledge of multiple community resources in and around their community and province
- Valid Driver's License and reliable insured vehicle
- Eligible to work in Canada
- Vaccinated against COVID-19
- Vaccinated against Influenza
