Registered Professional Nurse

Assess nursing care needs and prepare, implement, and evaluate care plans while performing approximately 18-20 client visits weekly. Serve as a Case Manager for a caseload of clients, formulating plans of care with physicians to address medical, psychosocial, and safety needs. Carry out therapeutic treatments, instruct patients or families on diet, health habits, and home treatments, and provide ongoing monitoring for status changes requiring care plan revisions. Make referrals to other disciplines, agencies, or county entities to maximize client health and independence. Complete required Medicare, Medicaid, and third-party payer documentation, maintain client records, and participate in quality assurance and continuing education. Maintain a valid New York State nursing license and driver's license for continued employment.
