Registered Practical Nurse – Transitional Care Program – Talent Pool

Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in an interdisciplinary team context. Collaborate in inter-professional rounds, care planning forums, and family conferences; provide mentorship to RPN students and new staff; participate in professional development and hospital-wide initiatives; ensure patient safety and adherence to policies and procedures. Requires valid registration with the College of Nurses of Ontario and a Diploma in Practical Nursing. Proficiency with Electronic Patient Records and computer literacy expected; commitment to continuing professional development and teamwork across a large health-care organization.
