Job Summary
Registered Practical Nurse (Specialized Outreach Services) provides psychiatric nursing care, mental health assessment, consultation and interventions for adults on an assertive outreach basis to people who are homeless or street-involved and experience mental health and/or addictions issues. The role includes managing a caseload, delivering education on mental and physical health, and may involve travel to other agency sites to provide nursing support as part of an integrated mobile regional team. Works within a multidisciplinary team reporting to the Manager, Adult Intensive Services.
Required Qualifications
- Degree/Diploma from an approved Registered Practical Nursing (RPN) Program
- Current registration with the College of Nurses of Ontario and eligibility to practice as a Registered Practical Nurse in Ontario
- 3 to 5 years of related experience in providing community-based mental health services
- Thorough knowledge of psychiatric nursing and medications
- Well-developed assessment and intervention skills
- Good organizational, interpersonal, and problem-solving skills
- Demonstrated understanding and sensitivity to social and cultural diversity
- Demonstrated ability to work collaboratively in a fast-paced and multi-disciplinary setting
- Excellent verbal and written communication skills
- Proficient with Microsoft Office, Outlook and various database software
- A valid driver’s license, vehicle and insurance
