Registered Practical Nurse (RPN) Team Lead, Assisted Living - Temporary, Full-time

Job Summary

Monitor and direct care delivery, coordinate transdisciplinary team services, and supervise RPNs within assigned geographical boundaries. Conduct regular team meetings and huddles to appraise Key Performance Indicators and ensure adequate staffing. Provide complete client assessments, implement care plan strategies, and document responses using established procedures. Deliver hands-on personal care, complete home visits for competency assessments, and assign urgent care requests including sick calls. Teach service providers using adult learning principles, resolve complaints, and advocate for client-directed care. Maintain a safe workplace compliant with provincial Occupational Health and Safety Act and VON policies. Requires a personal vehicle, current driver's license, insurance, and CPR certification. Full-time, 75 hours bi-weekly, days schedule with on-call rotation. Temporary position ending April 2027 for an existing vacancy.

Required Qualifications

  • Registered Practical Nursing (RPN) diploma
  • Minimum 5 years' experience in community based health care system
  • Registered with College of Nurses in province of practice
  • Current CPR Certification
  • Must have personal vehicle and possess both a current driver's license and proof of vehicle insurance
  • Current criminal background check including a vulnerable sector screen
  • Must be able to wear Personal Protective Equipment (PPE)
  • Must be able to lift, carry, bend, reach, kneel, and perform other movements emphasizing good body mechanics
  • Must be able to walk, sit, stand, and climb stairs throughout the day
  • Must be able to perform fine hand movements
  • Proficient nursing skills with the ability to read and interpret technical and clinical instructions
  • Strong observation and assessment skills
  • Advanced computer skills including Microsoft Office Suite and Windows OS
  • Strong verbal and written communication skills
  • Interpersonal relationship building
  • Ability to problem solve and adapt nursing/unregulated service provider interventions as appropriate
  • Ability to work collaboratively as a member of an interdisciplinary team
  • Experience in hands on personal care
  • Coaching, mentoring and conflict resolution
  • Team Lead or supervisory experience

Desired Qualifications

  • Team Lead or supervisory experienced preferred
  • Clinical practice experience preferred
  • Health teaching and facilitation experience preferred
