Job Summary
Provide ethical, safe nursing care during scheduled community visits within your scope of practice; develop and monitor individualized care plans using assessment tools; engage patients to build rapport and trust; maintain comprehensive patient records; travel to patient homes within designated area; collaborate with RNs to support complex cases and ensure continuity of care; opportunities for flexible scheduling with part-time or full-time options; formal training, support, and AI-assisted recruitment processes are in place; ParaMed is Canada's leading home health care provider supporting patients wherever they call home.
Required Qualifications
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver’s license and access to a vehicle
- Experience in community care is an asset
- Willingness to travel to patient residences within a designated area
- On-call coverage including after hours
