Job Summary
Conduct scheduled community nursing visits to care for patients of all ages, utilizing appropriate assessment tools to evaluate individual needs. Execute and monitor care plans while adjusting as required, maintaining professional patient records for all encounters. Build rapport and trust through collaborative approaches to help patients remain comfortable in their homes. Provide ethical and safe nursing care within your scope of practice, supported by RNs for complex cases. Requires CNO registration, valid driver's license, and willingness for on-call coverage including after hours.
Required Qualifications
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license
- Access to a vehicle
- Required on-call coverage, including after hours
