Job Summary
Provide transitional care to alternative level of care patients awaiting community transition, focusing on patient and family-centered care within a multidisciplinary team. Manage patients with aggressive behaviors while demonstrating critical thinking, prioritization, and independent problem-solving skills. Deliver physical assessments and maintain compliance with health and safety legislation and mandatory education. Maintain well-developed clinical skills through recent inpatient rehabilitation or medical/surgical experience and ongoing professional development.
Required Qualifications
- Current Certificate of Registration with the College of Nurses
- Current BLS certification
- Recent experience managing patients with aggressive behaviours
- Recent related inpatient rehabilitation and/or inpatient medical/surgical experience with evidence of well developed clinical skills
- Excellent physical assessment skills
- Proficient computer skills
- Demonstrated excellent critical thinking and problem solving skills
- Demonstrated ability to work effectively within an interprofessional team as well as independently
- Well developed organization, communication, interpersonal and teaching skills
- Evidence of ongoing professional development, continuing education and leadership experience
- Demonstrated good attendance and performance records with the ability to maintain these same standards
- Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required
- RNAO membership is preferred
