Job Summary
Conduct comprehensive assessments, develop care plans, and lead discharge planning for patients on the Medicine and Transitional Bed Unit. Collaborate with the interprofessional team to provide patient education and ensure continuity of care throughout the inpatient stay. Perform daily duties including medication administration, vital signs monitoring, and wound care while maintaining strict adherence to health and safety legislation. Work 12-hour day and night shifts (0730-1930 & 1930-0730) across weekdays, weekends, and statutory holidays.
Required Qualifications
- Graduate from a recognized Registered Practical Nursing diploma program
- Current RPN registration with the College of Nurses of Ontario
- Current BLS certification
- Recent medical/surgical experience
- Excellent patient assessment skills
- Demonstrated excellent critical thinking and problem solving skills
- Well developed organization, communication, interpersonal and teaching skills
- Proficient computer skills
- Evidence of ongoing professional development, continuing education and leadership experience
- Demonstrated good attendance and performance records
- Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required
- 12 hr day and night shifts (0730-1930 & 1930-0730) on weekdays, weekends and statutory holidays

- Must be able to work statutory holidays
