Job Summary
Conduct comprehensive patient assessments, develop care plans, and lead discharge planning within the Medicine and Transitional Bed Unit. Collaborate with the interprofessional team to provide patient education and ensure continuity of care throughout the inpatient stay. Manage 12-hour day and night shifts across weekdays, weekends, and statutory holidays. Maintain current RPN registration and BLS certification while adhering to strict safety protocols and organization policies.
Required Qualifications
- Graduate from a recognized Registered Practical Nursing diploma program
- Current RPN registration with the College of Nurses of Ontario
- Current BLS certification
- Recent medical/surgical experience
- Excellent patient assessment skills
- Well developed organization, communication, interpersonal and teaching skills
- Proficient computer skills
- Evidence of ongoing professional development, continuing education and leadership experience
- Demonstrated good attendance and performance records with the ability to maintain this same standard
- Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required
- 12 hr day and night shifts (0730-1930 & 1930-0730) on weekdays, weekends and statutory holidays
