Job Summary
Plan, direct, and evaluate patient and staff care while implementing standards of care and developing field personnel. Complete patient onboarding, develop and maintain Care Plans, and ensure the level of care is safe, appropriate, and compliant with regulatory standards. Communicate with Ontario Health at Home regarding issues, consult with patients and families, and maintain accurate documentation including shift reports and risk codes. Respond to client requests, investigate complaints and incidents, and conduct patient satisfaction calls. Support and advocate for field staff by approving special functions, performing supervisory visits, and overseeing scheduling coordinators. Supervise Personal Support Worker performance, ensure adherence to quality assurance and infection control policies, and provide ongoing coaching and mentoring.
Required Qualifications
- Registered Practical Nurse with current unencumbered licensure to practice professional nursing in the Province of Ontario
- Thorough knowledge of Personal Support Workers processes and practices
- Ability and willingness to travel in Ottawa and surrounding area
- Ability and willingness to be on call 1 week (7 days) every five weeks to provide phone support
- Must be able to complete client home visits and have access to a reliable vehicle
Desired Qualifications
- Previous experience with Ontario Health at Home programs and processes
- French proficiency
- Excellent customer service skills and ability to deal tactfully with patients, staff and community in English and French
