Evaluate functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to promote independence, safety, and quality of life for residents. Collaborate closely with an interdisciplinary team, educate families and caregivers, document progress accurately, and contribute to discharge planning within a senior living community. Provide compassionate, resident-centered rehab services in a supportive, team-focused environment.
