Job Summary
Conduct comprehensive medical oversight for individuals with intellectual and developmental disabilities by developing care plans based on patient assessments and collaborating with physicians. Update medical diagnoses and care changes in the electronic health record, participate in pre-admission screening and discharge planning, and review nursing service plans semi-annually. Advocate professionally for community resources and ensure the health and physical well-being of patients while managing cultural differences. Requires a New York State RN license, one year of community health experience, and CPR/First Aid training.
Required Qualifications
- Possess New York State RN license
- Have 1 years' experience in community health care services
- Be trained in CPR and First Aid
