Job Summary
Evaluates patients of all ages for wound, ostomy, and continence needs; collaborates with healthcare providers to enable timely assessment and inter-professional interventions. Utilizes the nursing process to develop, implement, and document care plans for patients with skin breakdown risks, performing follow-up to evaluate outcomes. Designs patient education on wound and ostomy care tailored to developmental levels and cultural values, coordinates with physicians and nursing staff, and develops discharge plans with appropriate follow-up. Implements evidence-based care programs, evaluates resource utilization, and educates the broader healthcare team on wound management standards. Works in a constant state of alertness while performing duties as assigned.
Required Qualifications
- Graduate of accredited school of nursing or education equivalency for licensing
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Registered Nurse (RN) - Missouri Division of Professional Registration
- Registered Nurse (RN) Issued by Compact State
- Registered Nurse (RN) - Oklahoma Board of Nursing (OBN)
- Registered Nurse (RN) - Wisconsin Department of Safety and Professional Services
