Job Summary
Conduct medical assessments for individuals with developmental disabilities, acquired brain injuries, and substance abuse issues to determine appropriate community placement. Develop and implement service plans, review care protocols, and administer medications while ensuring all records and reports meet regulatory standards. Oversee high-risk medical conditions, provide medical skills training to staff and families, and coordinate transportation to appointments. Participate in Individual Health Plan meetings and attend on-call rotations as needed.
Required Qualifications
- Graduate of an accredited RN program
- One year experience working with medically complex individuals
- Current state RN License
- Current driver's license, car registration and auto insurance if providing transportation to individuals receiving services
- Current CPR/First Aid certification
- Wound Care Certification or willingness to obtain certification within 6 months of hire
- Tasks that do involve exposure to blood, body fluids, or tissues are an expectation of employment
- Exerting up to 100 pounds of force occasionally
- Exerting up to 50 pounds of force frequently
- Exerting up to 20 pounds of force constantly
