Job Summary
Conduct initial and ongoing patient assessments, including physical exams and medical history, while developing, implementing, and revising care plans based on patient needs. Administer medications and treatments as prescribed by physicians, providing direct patient care, counseling, and health education. Coordinate discharge planning and necessary rehabilitative care, serving as Case Manager when assigned to ensure seamless patient flow. Prepare clinical notes, update physicians regularly, and report patient condition changes to obtain necessary orders. Collaborate with healthcare professionals to coordinate care and participate in on-call duties as required. Ensure necessary medical equipment and services are arranged for patient care.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in state
- CPR certification
- Bachelor's degree
- One (1) to two (2) years of recent acute care experience in an institutional setting and home setting
- One (1) year of home health care experience preferred
- Excellent observation skills
- Verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Visual acuity and hearing to perform required nursing skills
- Able to lift, position or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling or crouching
Desired Qualifications
- One (1) year of home health care experience preferred
- Management experience not required
