Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations, and manage acute and post-acute care for clients in home settings. Perform wound care, IV therapy, medication reconciliation, and safety monitoring while documenting at the point of care. Work independently to make real-time clinical decisions and prioritize urgent visits across Dauphin, Cumberland, York, Lancaster, Adams, and Franklin County. Requires a current Pennsylvania nursing license, one year of recent hands-on medical experience, and ability to adapt to varied home environments. Earn competitive pay ranging from $55.00 to $65.00 per point with flexible scheduling.
Required Qualifications
- Home Health experience
- A current nursing license in good standing in the state of Pennsylvania
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)

- Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)

- Perform comprehensive wound assessment, care, and documentation
- Ability to work independently while knowing when to escalate concerns

- Ability to manage a dynamic visit schedule and prioritize urgent visits
- Efficient with point-of-care documentation and technology
