Job Summary
Provide care and services for patients residing in private homes, Skilled Nursing Facilities, and assisted living facilities as part of an interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate community resources within Case Management, Admissions, or Resource roles. Manage visits, documentation, and urgent needs by prioritizing and triaging multiple concerns while communicating via phone, email, and meetings with healthcare professionals. Requires current RN licensure, ability to drive across the service area, and proficiency in electronic medical records systems.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Reliable transportation
- Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system
Desired Qualifications
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Work closely with patients on a 1-1 level but get strong support from a patient care team
- Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations
