Job Summary
As a Registered Nurse in home health, provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws and agency policies. Coordinate total plan of care with other health care professionals, perform clinical nursing assessments, verify patient eligibility and medical necessity, develop and revise individualized care plans, and ensure the plan of care meets patient needs with physician approval. Function in diverse home environments and communicate effectively with patients, families, and care teams to support continuity of care.
Required Qualifications
- Current and unrestricted RN licensure in the state of Oregon
- Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
- Preferred CPR certification or ability to complete within 90 days of hire
- 1+ years of RN experience
- Able to work independently
- Good communication, writing, and organizational skills
